A proxy directive is an instrument used to give another person the power to make your healthcare decisions.

Advance Directive / Health Care Proxy / Power of Attorney for Healthcare

An advance directive is sometimes called a power of attorney for healthcare or a healthcare proxy. An advance directive is similar to a proxy directive, but it is only effective when you are unable because of incapacity to make healthcare decisions for yourself. Once you regain the ability to make healthcare decisions for yourself, an advance directive is no longer effective.
